Please print and follow the instruction. This is a purchase Agreement between the below listed person and Please print this agreement and fill it out. Color:_____________________________________ If picking puppy up, date of pick up planned,_____________________________________________________ Airport of Choice:__________________________________________________________________________ Payment method:__________________________________________________________________________ Health Agreement: Please make sure that you agree to all terms before you purchase a puppy from us.If we do not recieve agreement from you & you take possession  a puppy form us then you are legally bound to the terms of this agreement!  Notice: Please remember that if you purchase a small breed puppy that they do require extra care, they must be fed more often to make sure they to not have what is commonly known as a" sugar drops". Make sure you are ready for this commitment prior to adoption. We will not be held responsible for puppies that have  Internal Parasities,Coccidia, Kennel cough, Parvo, Distemper  after they are picked up. Puppies that are paid for on a payment schedule: Should the puppy of your choice become ill prior to shipping we will have the puppy treated and will only ship once we are 100% sure this puppy is healthy. In the event the puppy should die ,Seller has the right to replace your puppy with a puppy of the same breed and quality. You will be given choice of the puppies available at that time. No cash refund will be given under any circumstance; and Buyer will be responsibile for all shipping costs to return puppy. This puppy is guaranteed to be in good health, to the best of our knowledge, at the time of sale and or shipping. This puppy has had vaccinations listed in a pet health record to protect against illness. Deworming treatments have been given regularly as shown on said record, however there is no way to guarantee that this puppy is completely free of intestinal parasites and will need to be given a regularly scheduled deworming as recommended by a licensed veterinarian.

There are certain disorders that may be caused by environmental factors, for example: Allergies, Thyroid dysfunction, demodectic mange, and other autoimmune disorders that are not covered by this guarantee.

The buyer of this puppy has 48 hours from the date of receiving your puppy to have this puppy examined by a licensed veterinarian. Should the veterinarian find that the puppy has a life threatening genetic defect this puppy will be exchanged for another puppy of the same breed when it becomes available written statement from the veterinarian is required. This statement must be faxed to us within the 24 hours to guarantee replacement of your puppy. Luv Country Puppies has the right to require a second opinion of a veterinarian of our choice to verify findings. Under no Circumstances will a cash refund be given or will accept responsibility for any medical expenses after the puppy leaves our possession. It is agreed that no guarantee or representation has been made other than what is written in this agreement. Vaccinations - Isolation is the best form of protection for a new puppy until it has had his/her 12 week vaccination and he/she can begin building its own immunity. Parvo and other problems can be brought into an environment on the bottom of shoes, on clothing (airborne virus) and by other animals, or by humans who have been exposed to infected animals. If your puppy is under 16 weeks of age it is a good idea to keep him/her away from other animals as much as possible, ask your friends to wash their hands before handling your new addition. Another dog may look normal and be perfectly healthy itself, but can be a passive carrier of a virus or bacteria that your puppy has no immunity to. An ounce of prevention is worth a pound of cure when it comes to exposing your new puppy to outside dogs.
